Death by CAPA

This is a decision tree showing a risk-based approach to addressing quality issues. Trend analysis is appropriate for predicable issues with existing process controls. Quality improvement plans are appropriate for large projects where the root cause is unknown and in-depth analysis may be required. Anything in-between should be addressed with a CAPA.
